POSITION OBJECTIVES

The Stronger Families Worker will support families to consider the changes that need to occur for children to remain safe in their own homes. The position will provide intensive outreach and in-home support to families. The Placement Prevention Program is a new project with the aim of increasing the health, wellbeing and safety of children and young people who might otherwise have been at risk of entering the Out of Home Care system. The Stronger Families Worker will participate in comprehensive family assessments that identify the health, wellbeing and safety needs of the children involved, and that identify both the capacity and constraints of the families to make necessary changes.

PROGRAM DUTIES

Client Support

Participate in comprehensive family assessments that identify the health, wellbeing and safety needs of the children involved, and that identify both the capacity and constraints of the families to make necessary changes Support the strengthening of family connections, promoting placement prevention and promoting the safety of children, young people and parents To participate in the development of comprehensive family plans using a wraparound approach and participation in ongoing review and evaluation of family plans To actively participate in team work in a manner that ensures a co-ordinated approach to services for children, young people and their families To work in conjunction with existing services to ensure that referrals are responded to in a way that acknowledges the unique needs of each referral and situation by matching resources to referral information. To support a comprehensive assessment process that results in both high quality information for decision making and an increased understanding by the referred family about their situation, capacity and choices. Provide crisis support to clients engaged with the Stronger Families programs as needed Ensuring that every plan is reviewed in the stated timeframe and appropriate actions are taken to ensure ongoing progress Provide confidential client support to individuals and families to improve the safety, stability and developmental outcomes for child/ren Support individuals and families wanting to make positive lifestyle changes

KEY SELECTION CRITERIA

  • Knowledge of child and adolescent development and strategies for working with vulnerable children, young people and their families
  • Experience and skill in rapid engagement and goal setting
  • Experience in assessment and development of measurable family care plans
  • Excellent communication and interpersonal skills
  • Understanding of aboriginal culture
  • Ability to effectively and efficiently plan and organise own workload
  • An ability to manage stressful situations
  • Data collection and record keeping skills
  • The ability to work as part of a team and a strong commitment to collaborative working relationships
  • Demonstrated negotiation and advocacy skills
  • Extensive conflict resolution experience
