Job Description
Guardian Network is a fast growing and innovative in-home aged and disability care organisation dedicated to providing the best possible care for those with disabilities and to the senior members of our community. Guardian Network aims to provide the best in home care for clients who wish to retain their autonomy and remain in their own homes in their latest stages of life. Due to recent growth and exciting new position has been created to work within a leading aged and disability care organisation on the eastern suburbs of Melbourne.Basic Function:
  • To match and roster our professional care team members with our clients on our after hours emergency services. This work can be exceptionally complex and challenging, when considering the individual circumstances of our clients.

  • Understanding and experience working in the aged care sector

  • Strong organizational and planning skills with the ability to manage conflicting priorities.
  • Ability to deal with emergency situations and deal with urgent request
  • A strong, empathetic manner
  • Ability to establish priorities, follow instructions, work to tight, non-negotiable timelines, autonomously or as a part of a management team
  • Well-developed problem-solving skills
  • Excellent memory and recall
Duties:
  • Communicate appropriately with supported individuals, their families and / or funding bodies and other relevant stakeholders.
  • Provide rostering of services ensuring all roster information is updated promptly and changes entered onto iCare Health
  • Provide on call coordination on a rotational roster set by the Client Services Manager
  • Ability to be on call for the hours of 7pm to 7am Monday to Friday and 4pm to 7:30am on Saturdays, Sundays and Public Holidays.
  • Schedule staff for urgent requests and leave coverage based on requests for skills mix, availability, and the geographic proximity to minimize travel, ensuring suitable match of carer skills/personality is given
  • Complete after-hours spreadsheet with comprehensive and accurate information.
  • Complete handover in a timely manner and accurately handover any relevant information.
  • Escalate emergency situations to Client Services Manager
  • Accurately record consumer services using the iCare Health system
  • Accurate and timely documentation and communication of any client issues or concerns
  • Advise case managers/ after hour services of changes and concerns reported by staff
  • Develop clear communication with careers
  • Maintain high standards of practice in dealings with supported individuals, staff, and other stakeholders.
  • Assist to develop and actively promote mechanisms for eliciting feedback.
  • Identify and contribute to the continuous development and implementation of process and systems to improve and enhance service delivery.
  • Undertake appropriate action in accordance with Guardian Network policies following reported grievances.
  • Represent Guardian Network to supported individuals, staff, external agencies, and the public in a positive manner always
  • Provide timely and accurate information to the Client Services Manager regarding service delivery issues.
  • Encourage clear, open, honest, and constructive communication amongst all staff.
Essential Requirements
  • Experience in provision of services in Aged Care in Home Support, would be advantageous
  • Current police check
