Permanent part-time position - 25 hours / week

Workplace: Welland

Salary: $32.74 to $39.44/hour (bachelor’s degree); $34.65 to $41.74/hour (Master’s degree)

Under the direction of the Caring for my COPD Coordinator, the incumbent will always work according to the principles of health promotion and the determinants of health. He / she will offer psychosocial evaluations and interventions to a primarily geriatric population as well as develop and implement brief intervention plans. In addition, he / she will provide the appropriate referrals for clients with issues requiring long-term interventions and will participate in education.

RESPONSIBILITIES :

  • Provide assessment and individual mental health intervention services to clients of the Caring for my COPD program
  • Works collaboratively with other Caring for my COPD team members
  • Develop and lead educational and therapeutic groups to improve the quality of life for clients with obstructive lung disease
  • Participate in the development and application of intervention models that ensure quality services
  • Liaise with community agencies to improve access to appropriate services in a timely manner
  • Prepare reports, maintain records and maintain program statistics
  • Provide individual interventions by using diverse therapeutic approaches such as: behavioral and cognitive behavioral therapy, ensuring approaches are evidence based
  • Gather and analyze the necessary data to properly identify needs. Determine the services and resources required to meet the identified needs and refer to internal and external services as needed
  • Offer individual help sessions aimed at quitting smoking
  • Provide nicotine replacement therapy within the parameters established by CAMH as part of the STOP initiative
  • Developing and facilitating group educational sessions for the Caring for My COPD program.
  • Completing initial and follow-up assessments of each participant
  • Performing administrative tasks if needed (reminder calls, preparing schedules, preparing documents, etc.)

 

 SPECIFIC REQUIREMENTS:

  • Bachelor's degree or Master's degree in Social Work or a related field from a recognized institution
  • Member in good standing of a Professional College (OCSWSSW or CRPO ) or in process of
    registration
  • A minimum of three years of relevant experience
  • Developed sense of organization and in-depth understanding of mental health and addiction issues
  • Ability to handle crises effectively
  • Knowledgeable on the mental health community resources and other resources available in Niagara
  • Openness to the customs and needs of diverse populations
  • Good communication and interpersonal skills
  • Commitment to working cooperatively with clients and members of the interdisciplinary team
  • Able to use basic computer software
  • Certification Training Enhancement in Applied Cessation Counselling and Health (TEACH) from Center for Addiction and Mental Health (CAMH) an asset
  • English and French skills (oral and written) are assets
  • Experience working with geriatric population an asset
  • Experience providing CBT education an asset
  • Have a valid driver's license and have access to a motor vehicle
  • Provide a clean Police Vulnerable Person Criminal Record Check (PVSC)
  • Proof of COVID-19 vaccination (or a medical bill of exemption) will be requested at the time of hire.

Il/elle offrira des évaluations et des interventions psychosociales à une population principalement gériatrique, et élaborera et mettra en œuvre des plans d'intervention brève. En outre, il / elle fournira les références appropriées pour les clients dont les problèmes nécessitent des interventions à long terme et participera à l'éducation.
