<p><em><strong>DIRECT SERVICE PERSONNEL</strong></em></p> <p><strong>PURPOSE OF YOUR JOB POSITION</strong><br /> The primary purpose of your job position is to provide habilitation training and daily care to the Individuals in accordance with Federal, State, and local standards, as well as our established policies and procedures, focusing on recommendations contained in the Individual Habilitation Plan.</p> <p><strong>MAJOR DUTIES AND RESPONSIBILITIES</strong><br /> 1. Conduct daily observations and documentation.<br /> 2. Promote teamwork by assisting co-workers in the care of individuals regardless of staff assignments.<br /> 3. Ensure that assigned documentation is current and appropriately completed.<br /> 4. Comply with written behavior programs and document behavioral incidents in individual client files.<br /> 5. Ensure that individuals are well-groomed and appropriately attired for the weather and that clothes are of proper fit and presentable.<br /> 6. Assist in the dining area during mealtime to ensure an individual&#39;s needs are being met through<br /> monitoring and structured programs.<br /> 7. Ensure normalized training and interaction during meal periods with individuals.<br /> 8. Assist individuals in self-medication programs.<br /> 9. Monitor individuals when necessary during activities and complete documentation.<br /> 10. Assist individuals in shopping for personal care items.<br /> 11. Complete home and individual laundry as directed.<br /> 12. Maintain a clean and safe environment through the performance of daily cleaning.<br /> 13. Assist individuals in doctor appointments, dentist appointments, etc.<br /> 14. Monitor individual client supplies for cleanliness and appropriateness.<br /> 15. Attend Direct Services Personnel Certification classes as assigned by House Manager/QIDP.<br /> 16. Maintain the CONFIDENTIALITY of all pertinent individual care information to assure<br /> patient rights are protected.<br /> 17. Maintain the CONFIDENTIALITY of all pertinent home records.<br /> 18. Perform other specific duties if requested regardless of routine schedule within the training and capabilities.<br /> 19. Perform other special duties as designated by House Manager, QIDP or Executive Director.</p> <p><strong>EDUCATIONAL REQUIREMENTS</strong><br /> Must possess, as a minimum, a high school diploma or equivalent; special exceptions may be made on a case by case basis. Must complete required Direct Service Personnel Certification Classes</p> <p><strong>ESSENTIAL FUNCTIONS</strong><br /> ? Must be able to accept a change of job position and/or work schedule resulting in overtime without prior notice due to the health care provided by our facility to persons with developmental disabilities.<br /> ? Must attend all in-service training and staff meetings as required by the House Manager,<br /> QIDP or Executive Director.<br /> ? Must have knowledge of and adhere to client rights and regulations regarding privacy issues.<br /> ? Follow home vehicle policies regarding cleanliness, no smoking, recording of mileage, etc.<br /> ? Assist in training new personnel regarding the house, its policies, procedures, and to his/her job position and duties.<br /> ? Required to assist non-ambulatory clients in transferring (equal to or greater than fifty (50) pounds for a short duration), sitting, standing, bending, lifting and moving intermittently during work hours.<br /> ? Required to assist partially ambulatory individuals in walking using gait-belt.<br /> ? Required to assist individuals with their self-care skills such as bathing, toileting, eating, etc.<br /> ? Required to have a valid driver&#39;s license to operate a home vehicle and to be considered insurable based on criteria by the home insurance carrier.<br /> ? Must not have an infectious disease that would pose an undue risk of infection to themselves, other employees, individuals, or visitors. (Illinois Administrative Code Chapter 1 Section 350.675)<br /> ? Must provide a home with health inventory, which includes immunization status, history of the infectious condition, hepatitis, dermatologic conditions, or chronic draining infections or open wounds.<br /> ? Must be of good temperate habits, good moral character, be honest, reliable, and trustworthy.<br /> ? Must be able to speak and understand the English language or a language understood by a substantial percentage of the individual residents.<br /> ? Must provide evidence of employment or occupation, if any, and residence for two years prior to initial employment as a DSP.<br /> ? Must not have been convicted or had a finding entered against you of abuse, neglect, or mistreatment of the elderly, disabled, or children.<br /> ? Must be willing to undergo initial and annual background checks<br /> ? Must demonstrate the required level of education competency through testing as set forth by Federal and State standards.<br /> ? Must follow policies of facility and company<br /> ?
